Enhanced Care Coordination (ECC) ProgramProgram Overview
The Enhanced Care Coordination (ECC) Program is a free benefit that aims to provide comprehensive, patient-centered care coordination to optimize health outcomes and improve quality of care. The program works with Medicaid recipients in the Temporary Assistance to Needy Families (TANF) and Aid to the Permanently and Totally Disabled (APTD) eligible populations.
In addition to ensuring coordinated medical and behavioral
health services and individualized health care plans focused on prevention for Medicaid
recipients with complex health care needs, the program components also include:
fostering the concept of a Medical Home, implementation of standardized, evidence-based
clinical guidelines for decision making, provider outreach and support, and quality
improvement via benchmarking, establishing performance standards and outcomes measurement.
